Pharmacokinetics

Absorption: Perphenazine has an oral bioavailability is 100%, pre-systemic (first-pass) metabolism accounts for 65% (±5%).

Distribution: Perphenazine has a volume of distribution is 20 l/kg.

Elimination: it is metabolised via Hepatic, plasma half-life is 9.5 hr, renal excretion is 1 %.

Indications

Perphenazine is primarily indicated in conditions like Bipolar disorders, Mania, Nausea and vomiting, Psychosis, Schizophrenia.

Perphenazine is also indicated for secondary conditions like Post-operative nausea and vomiting.

Side Effects

Perphenazine in overdosage may give rise to further complications which include Numbness , Retrocollis , Slurred speech , Aching, Hyperreflexia , Dystonia , Trismus , Motor restlessness , Tachycardia , Cardiac arrhythmias, Urticaria , Hypotension , Jaundice , Extrapyramidal symptoms, Gynacomastia , Anaemia .

Perphenazine produces potentially life-threatening effects which include Coma, Neuroleptic Malignant Syndrome , Extrapyrimidal symptoms, Stupor . Careful monitoring is needed with proper medical management.

The signs and symptoms that are produced after acute overdosage of Perphenazine are Miosis, Tachycardia , Confusion , Hypotonia, Tremor, Hypotension, Hypothermia, Restlessness .

Perphenazine is known to cause more or less tolerable side effects which are usually transient. These include Blurred vision, Constipation , Dry mouth, Miosis , Mydriasis, Pain , Sedation .

Warnings / Precautions

Perphenazin should be used with caution or not at all in patients with impaired liver, kidney, cardiovascular, cerebrovascular, and respiratory function and in those with angle-closure glaucoma, a history of jaundice, parkinsonism, diabetes mellitus, hypothyroidism, myasthenia gravis, paralytic ileus, prostatic hyperplasia, or urinary retention. Care is required in patients with epilepsy or a history of seizures. Regular eye examinations are advisable for patients receiving long-term Perphenazine Enantate and avoidance of undue exposure to direct sunlight is recommended. Should be used with caution in the presence of acute infection or leucopenia. Blood counts are advised if the patient develops an unexplained infection or fever. Generally not recommended late in pregnancy; such use may be associated with intoxication of the neonate. Photosensitization may occur, caution patients against exposure to sunlight or UV light. Discontinue use at least 48 hrs. before myelography.

Interference in Pathology

Perphenazine may interfere in the diagnosis of False +ve tst for urobilinogen, amylase, uroporphyrins, porphobilinogens , Abnormal LFTs .
