Dobutamine is a synthetic, direct acting catecholamine that is a ß1 receptor agonist. Dobutamine (HCl) is available as racemic mixture. Chemically it resembles dopamine. It provides additional pumping strength by stimulating the heart muscle and effective to treat low output acute heart failure

Indications

Dobutamine (HCl) is primarily indicated in conditions like Acid reflux disease (long-term management), Cardiac surgery, Cardiogenic shock, Cardiomyopathies, Heart failure, Hypotensive shock, Inotropic support in infarction, Septic shock.

Dobutamine (HCl) is also indicated for secondary conditions like Positive end expiratory pressure ventilation.

Side Effects

Dobutamine (HCl) in overdosage may give rise to further complications which include Tissue necrosis .

Dobutamine (HCl) produces potentially life-threatening effects which include Myocardial Ischemia , Tachycardia, Cardiac arrhythmias, Hypertension, Hypotension, Myocardial ischemia, Extravasation. Careful monitoring is needed with proper medical management.

The signs and symptoms that are produced after acute overdosage of Dobutamine (HCl) are Hypertension, Peripheral vasoconstriction, Myocardial ischemia, Arrhythmias.

Dobutamine (HCl) is known to cause more or less tolerable side effects which are usually transient. These include Breathlessness , Bronchospasm, Chest pain, Fatigue, Headache, Nausea, Non-specific chest pain, Palpitation, Paresthesias, Reaction at injection site, Vomiting .

Warnings / Precautions

Correct hypovolemia with suitable volume expender before starting treatment with dobutamine in hypovolemic patients. Continuously monitor ECG, blood pressure and cardiac output. It should be used with extreme caution in patient with acute myocardial infarction. It is classified as pregnancy category C and should be avoided during pregnancy unless the potential benefit outweighs the isk to the fetus. Avoid in arrythmias, severe hypotension.

Interference in Pathology

Dobutamine (HCl) may interfere in the diagnosis of Reduction in serum potassium concentration.
