Mepivacaine (HCl) is amide type local anesthetic. It consists of an aromatic ring connected by an intermediate chain, including an ester or amide to an ionizable group, usually a tertiary amine. The actions and effects similar to that of lidocaine. It works by preventing nerves from transmitting painful impulses to brain which creates temperory anesthesia.

Distribution: Mepivacaine has a plasma protein binding is 78 %.

Elimination: it is metabolised via through Liver, plasma half-life is 2 - 3hr (adults), 9hr (neonates),, renal excretion is 5 - 10 % (unchanged).

Indications

Mepivacaine is primarily indicated in conditions like Anesthesia, Caudal anaesthesia, Epidural anaesthesia.

Mepivacaine is also indicated for secondary conditions like Infilteration anaesthesia, Nerve block.

Contraindications

Mepivacaine is contraindicated in conditions like Hypersensitivity.

Side Effects

Mepivacaine in overdosage may give rise to further complications which include Cardiac arrhythmias, Convulsions , Allergic reactions , Chest pain , Tremors .

Mepivacaine produces potentially life-threatening effects which include Coma, Convulsions , Respiratory Failure , Cardiac Arrest . Careful monitoring is needed with proper medical management.

The signs and symptoms that are produced after acute overdosage of Mepivacaine are Skin eruptions.

Mepivacaine is known to cause more or less tolerable side effects which are usually transient. These include Anxiety, Backache , Blurred vision, Dizziness, Excitement, Headache, Nausea and vomiting, Nervousness, Numbness , Restlessness, Tremors.

Warnings / Precautions

Avoid mepivacain in patient with known hypersensitivity. It should be given with caution to the elderly, to debilitated, to children, patients with epilepsy, impaired cardiac conduction or respiratory function, shock or liver damage. It might be ototoxic and should not be instilled into the middle ear. The application of local anesthetics to the skin for prolonged period or to the extensive area should be avoided. Avoid alcohol. Donot drive because Mepivacaine may cause dizziness.
