Ethionamide is antimycobacterial agent. Ethionamide is chemically related to isoniazid. Ethionamide is poorly water soluble and available only in oral form. Resistance to drug emerges rapidly when the drug is used alone. Therefore Ethionamide is always given in combination with other antituberculosis drug.

Absorption: Ethionamide has an oral bioavailability is 92.5% (±2.5%).

Distribution: Ethionamide has a plasma protein binding is 30%.

Elimination: it is metabolised via extensive by liver, plasma half-life is 1.8-2.4 hr, renal excretion is entire.

Indications

Ethionamide is primarily indicated in conditions like Prophylaxis of acid aspiration in obstetric patients, Tuberculosis.

Ethionamide is also indicated for secondary conditions like Multibacillary leprosy.

Contraindications

Ethionamide is contraindicated in conditions like Hepatic damage, Chronic resistant vaginal candidiasis.

Side Effects

Ethionamide in overdosage may give rise to further complications which include Encephalopathy , Convulsions , Hepatotoxicity , Hepatitis , Hypersensitivity reactions , Anxiety , Optic neuritis , Depression , Hepatotoxicity , Acute psychosis, Optic neuritis.

Ethionamide is known to cause more or less tolerable side effects which are usually transient. These include Abdominal pain, Acne, Alopecia, Anorexia , Deafness , Diarrhea, Dizziness, Goiter, Gynecomastia, Headache, Hypoglycemia, Hypotension , Impotence, Jaundice , Menorrhagia , Menstrual irregularity, Nausea, parasthesia, Purpura, Salivation , Skin rash, Stomatitis, Taste disturbance, Vomiting .

Warnings / Precautions

Ethionamide should be used with caution in patients with any allergy (especially drug allergy), liver disease or diabetes. Limit alcohol intake since alcohol can increase the drowsiness effect of this medication and also affect the liver. It should be used with caution when performing activities requiring alertness. This medication should be used only when clearly needed during pregnancy or lactation.

Interference in Pathology

Ethionamide may interfere in the diagnosis of Urinary Determination of Alkaline Phosophatase, Lactic dehydrogenase& Phosphate .
