Carbimazole used in the treatment of hyperthyroidism. Carbimazole depresses the formation of thyroid hormone.

Pharmacokinetics

Absorption: Carbimazole has an oral bioavailability is 95% (±5%), pre-systemic (first-pass) metabolism accounts for 100%.

Distribution: Carbimazole has a volume of distribution is 0.5 l/kg. plasma protein binding is 40%.

Elimination: it is metabolised via via liver & thyroid, plasma half-life is 3-5 hr, renal excretion is 90%.

Indications

Carbimazole is primarily indicated in conditions like Blocking-replacement, Graves disease, Hyperthyroidism.

Contraindications

Carbimazole is contraindicated in conditions like Nodular goitre, Sub-acute thyroiditis, Post-partum painless thyroiditis.

Side Effects

Carbimazole in overdosage may give rise to further complications which include Insulin autoimmune syndrome , Cholestatic jaundice , Lupus like syndrome , Nephrotic syndrome , Hepatic necrosis .

Carbimazole produces potentially life-threatening effects which include Agranulocytosis, Oropharyngeal bacterial infection . Careful monitoring is needed with proper medical management.

Carbimazole is known to cause more or less tolerable side effects which are usually transient. These include Arthralgia, Fever, Headache, Loss of hair, Nausea, Urticarial skin rash, Vasculitis.

Warnings / Precautions

Excessive dose of carbimazole (antithyroid) should not be used, because it may cause hypothyroidism or goiter Treatment should be discontinued immediately if there is any clinical or laboratory evidence of neutropenia. It is not recommended to use high dose during pregnancy or lactation.
